Welcome to the eLearning module Managing Diabetes in Primary Care.

This module provides an interactive learning experience that focuses primarily on managing diabetes in two specific domains of care: Justice Health and Forensic Mental Health Network and Community Health.

Objectives

• Apply basic diabetes management principles,
• Use effective communication to gather information for clinical decisions and provide effective chronic care management especially during transfer of care.

This course is designed for Justice Health & Forensic Mental Health Network and Community Health Clinicians and may qualify for up to 0.5 hrs of Continuous Professional Development.
